Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma informed care environment. Completes all reporting requirements to OCFS on a timely basis. Participate in community-based education. Coordinates group facilitation in areas served – Monroe, Schuyler, Steuben and Wayne counties. Supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma informed care environment.Essential Duties and ResponsibilitiesProvides supervision and coordination of the post-adoptive, post-guardianship and kinship services to Monroe, Schuyler, Steuben and Wayne Counties.Provide performance management, development of staff, and other supervision responsibilities as requested by the agency and leadership.Preparations of quarterly OCFS reports.Participation in annual monitoring visits.Provides direct services to clients and their families primarily through face-to-face interactions on assigned caseload.Services include but are not limited to counseling, training/education, advocacy, group development and facilitation, and completion of documentation required to support client situation.Completes assessments, formulates, implements, and monitors service delivery plans appropriate for client population supported by the program.Coordinates service delivery.Completes required evaluations and screenings to include but not limited to medical, educational, legal, law enforcement, vocational and mental health.Facilitates and encourage connection with appropriate cultural and community resources.Attends appointments to include those required by various courts, school and medical. May be required to provide transportation to client and family members attending appointments.Work collaboratively Human Services agencies and other service providers in Monroe, Schuyler, Steuben and Wayne counties.Assists clients with crisis such as lack of food, housing, abuse, domestic violence, etc.Represents program at county level client service plan meetings, if required.Attends and participates in local community events.Maintains knowledge of community resources available to client population.Provides outreach to counties to build up service area.Meets regularly with Department Director and Program Supervisor to evaluate and implement professional growth in practice and performance.Inform Director and Supervisor in a timely fashion about significant events related to client issues.Participate in required Agency/Department/Program meetings, ongoing staff trainings, and program development.Initiate professional development through attendance at outside conferences, in-service training, reading, and applying current, relevant literature, etc.Completes required record documentation in accordance with Program Policies and ProceduresMaintains documented client case records that include all significant client interventions.Prepares and submits required reports and statistics in a timely manner.Participates in CYF Department QA process.Participates in relevant Agency meetings and/or trainings.Actively pursues development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops.Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan. Including, but not limited to, supporting the Agency's Core Competency initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse.Travel may be required to Monroe, Schuyler, Steuben and Wayne counties depending on assigned caseload 25-50%.
