This job was posted by : For more information, please see: Case Management Job DescriptionCCM-4U, LLC team supports people to identify their medical, employment, financial, social, educational, and other goals, identify the services necessary to meet those needs including natural supports in the community and waiver providers, learn how to advocate for their goals and facilitate access to identified services to live as independently as possible in their community.CCM-4U, LLC believes in and promotes the dignity, worth and diversity of all people.The role of a community case manager (CCM) as a team member is to support people so they can live as independently as possible while assuring their health and safety requirements are met. This is a 4-step process. CCMs support people to:1. Identify their needs and desires2. Advocate for what they want3. Find resources they need4. Secure resources to meet their needs and desires5. Monitoring to assess an individuals safety, well-being, and stability, as well as monitoring the progress toward the goals identified in their PCP.CCM-4U, LLC strives to engage each person we provide services to in the step-by-step process known as the Person-Centered Process (PCP) to assist each person to identify their goals based on their needs and desires. We provide PCP and coordination of services as an independent agent. That means we only provide case management and Rep Payee services. No residential supports or direct support services. Other organizations provide case management and direct services that can be a conflict and limits choice. We assist in connecting individuals and families to community resources and coordinating multiple services. We monitor services to make sure individuals get what they need, when they need. The Case Manager is responsible for documentation in accordance with regulatory agencies such as the Department of Health and Human Services. The Case Manager represents the CCM-4U, LLC in the community and acts as a liaison with other agencies.
