The Clinical Pharmacy Specialist provides direct patient care services for patients with chronic illnesses, promotes health and wellness, and participates in innovative patient visits including transitions of care, patient education classes, and shared visits.
This is a Per Diem position and is scheduled as needed, based on business and patient volumes.

Essential Duties & Functions

  1. Provides pharmaceutical care services through direct interactions with patients and providers and works independently under individualized scope of clinical practice.
  2. Conducts face-to-face or virtual care to provide chronic disease state medication management services.
  3. Obtains comprehensive medical and/or medication histories from patients and/or their records and summarizes the significant findings as documented in the medical record to ensure safe and effective care.
  4. Reconciles patient medications at ambulatory care clinic visits.
  5. Performs physical assessments and triaging patients as needed.
  6. Documents encounters and interventions in the patient electronic medical record.
  7. Designs, implements, assesses, monitors, and documents therapeutic plans utilizing the most effective, least toxic and most economical medications under scope of practice.
  8. Initiates, modifies, renews, refills, or discontinues medication therapy using approved delegated procedures.
  9. Orders labs and other tests to help determine efficacy and toxicity of medication therapy using approved delegating procedures.
  10. Documents/reports adverse drug events (ADEs) and medication allergies per procedure.
  11. Practices pharmacy in compliance with all governing regulatory authorities.
  12. Reviews patient profile to ensure appropriate drug selection and dosage.
  13. Provides accurate, adequate drug information/education to patients, families, and providers on drug interactions, side effects, dosage, and storage of pharmaceuticals.
  14. Assesses medication adherence during the process of medication counseling.
  15. Demonstrates the ability to set priorities, work independently and cooperatively with others within and outside the department.
  16. Presents information (verbal and/or written) in a clear and concise manner.
  17. Serves as preceptor to students at affiliated schools of pharmacy and pharmacy residents (when available).
  18. Performs other duties as assigned.

Required Education, Certifications, Licenses, & Training (minimum requirements)

  1. Graduate of an accredited college/university of pharmacy (PharmD or B.S. with equivalent experience).
  2. Completion of ASHP accredited PGY-1 Pharmacy Residency.
  3. Valid license from the Board of Pharmacy of the State of Texas.
  4. Certification in Basic Life Support / First Aid / CPR.

Required Years of Experience (minimum requirements)

Desirable Education and/or Experience:

  1. Board-Certified Pharmacotherapy Specialist (BCPS) or Board-Certified Ambulatory Care Pharmacist (BCACP).
  2. 2+ years of community pharmacy experience.
  3. 2+ years of medication adherence counseling experience.
  4. Experience in HIV/AIDs, Geriatrics, and/or Pediatrics.
  5. Completion of ASHP accredited PGY-2 Pharmacy Residency in Ambulatory Care, Adult Medicine, Geriatric Medicine, or Pediatrics.
  6. Certification in immunization delivery or smoking cessation.

Required Knowledge, Skills, and/or Abilities (minimum requirements)

  1. Computer experience – Windows, Microsoft Word & Excel.
  2. Must be able to manage multiple deadlines and prioritize.
  3. Excellent communication skills and customer service skills.
  4. This job requires access to confidential and sensitive information, requiring ongoing discretion and secure information management.

Desirable Knowledge, Skills, and/or Abilities

Significant knowledge and expertise in advanced pharmacy practice, specialty medication therapy management, and evidence-based medicine in area(s) appropriate for clinical assignments.
