Care Navigators will ensure the execution of service plans by accompanying patients to medical appointments, promoting healthy living, providing social support, health education literature, conducting home visits, and community outreach, as well as ensuring engagement and obtaining health home consents. Care Navigator will adhere to evidence based guidelines.
