JWCH Institute, Inc. is a non-profit, Federally Qualified Health Center (FQHC) with a mission to provide comprehensive health care to low-income and homeless persons in the County of Los Angeles. Position Summary: Within the regulatory scope of practice and under the direction of the Director or Program Designee, the LVN assists in the provision of direct patient care by performing routine patient care procedures, technical and supportive functions in the clinical setting. Principal Responsibilities: Performs routine patient care functions as prescribed by established policies and procedures within the defined scope of education, training, and responsibilities. Utilizes and practices "basic assessment" (e.g., data collection) and documents data in accordance with JWCH's policies and procedures, as well as the standards of LVN practice. Contributes to the evaluation of the patient's individualized interventions related to the care plan and treatment plan. Executes basic patient care consistently with JWCH's policies and procedures and the LVN licensure laws of the State of California. Applies principles of aseptic technique and infection control as per policy and procedures. Consistently employs and reinforces proper body mechanics and appropriate personal protective equipment. Recognizes the need for emergency care, initiates appropriate procedures within the scope of practice, and promptly alerts the Nurse Manager and physician. Conducts patient education and supports patients' self-management goals following JWCH's policies and procedures. Delegates selected tasks to a medical assistant, consistent with their job description and JWCH's policies and procedures. Assists medical personnel with procedures and/or diagnostic exams. Maintains cleanliness and stocking of patient exam rooms and work areas. Documents relevant patient information, performed procedures, and patient responses according to established policies and procedures. Immediately documents all patient care in the medical record after performing any task. Assists in the maintenance, care, and handling of medical equipment. Upholds standards of cleanliness, organization, and an adequate supply level for assigned exam rooms or work areas. Demonstrates correct and safe technique in equipment use and cleaning based on specific manufacturers' instructions and the policy and procedure manual. Holds responsibility and accountability for adherence to organizational and department standards and policies. Observes departmental guidelines concerning absences and reports absences following departmental procedures. Identifies potentially unsafe systems/processes/situations and proactively reports them to the supervisor. Participates in assigned organizational programs/committees. Demonstrates effective working relationships and collaborates effectively as part of the team to contribute to the organization's goals and objectives. Performs other assigned duties. Requirements: Current and valid California LVN License. Current BLS card from the American Heart Association. At least one-year experience in an ambulatory care setting. Ability to communicate effectively orally and in writing. Experience in working with clients and staff from diverse socio-economic, ethnic and cultural backgrounds. Current and valid California Driver's License with current car insurance. Preferred: Bilingual (English/Spanish) All JWCH, Wesley Health Centers workforce members must be fully vaccinated against COVID-19 as a condition of employment.
