JOB RESPONSIBILITIES:

  • Serves on the interdisciplinary team.
  • Participates in the development of participant plans of care.
  • Directly supervises the Day Center Manager and assists this individual in the supervision of the personal care aides and certified nursing assistants in the day center.
  • Attends morning staff meetings, interdisciplinary team meetings and other meetings as requested.
  • Communicates changes in participant status to interdisciplinary team members.
  • Conducts initial, scheduled and unscheduled reassessments of participants as indicated and enters required information into medical records.
  • Develops specific programs of recreational and social activities for participants based on individual cognitive ability.
  • Implements and supervises the daily activity program at the PACE Center.
  • Plans and coordinates the provision of both group and individual activities.
  • Records, signs and dates progress notes in the participant medical records.
  • Serves as a resource to staff and coordinates volunteer activities at the Center.
  • Participates in the coordination of a 24-hour care delivery system.
  • Participates in the Quality Improvement program.
  • Completes all documentation in the medical records as required.
  • Is friendly, courteous and helpful to participants, staff and the public.
  • Acts only within the scope of his or her authority in performing duties.

SUPERVISORY RESPONSIBILITIES: The Activities Director provides direct supervision for the Day Center Manager and assists this position with the supervision of the personal care aides and certified nursing assistants working in the day center. The Activities Director supervises the Day Center Manager as he/she performs duties associated with the Interdisciplinary Team and the Plan of Care. Works closely with the Center Manager, Clinical Services Supervisor, Registered Dietitian and other supervisors to ensure oversight of the care received by the participants in the day center.

QUALIFICATIONS:

Two years or more experience in a social or recreational program for frail or elderly people.

One or more years’ experience working full-time in an activity program, preferably in a health care setting.

Certified Nursing Assistant preferred; Certified Personal Care Assistant required.

Experience working as a member of a team and as the coach of a team is helpful.

Pleasant personality and positive outlook.

Ability to interact calmly with people in difficult and unusual situations.

Ability to use a computer – will need to enter data into electronic health records.

Medically cleared for communicable diseases with up-to-date immunizations before participant contact.

Criminal history records check at hire.

Drug screen at hire.
