Care Coordinator-Wellness Exams and Immunizations:
The Care Coordination is responsible for the delivery of patient care services for children through coordination, implementation, and supervision of patients.
Position Summary:
With a Quality , Customer First , and Compassionate approach, The Care Coordinator-Wellness Exams and Immunizations will:
  • Case management for children for children up to 21 years of age. This includes but not limited to registration, scheduling, connecting to resources, tracking/scheduling wellness exams, and that IZ's are up to date.
  • Identify patient needs, current services, and available resources and connect the patient to services and resources to meet established goals.
  • Provide case management to assigned patients.
  • Provide resources for legal services, financial, employment and housing.
  • Maintain current working knowledge of Medicare, Medicaid, SSDI, SSI and other program benefits/criteria: particularly as they pertain to low-income patients.
  • Assist patients with applications for funding towards medical expenses (i.e., hearing aids, dentures, and medical equipment).
  • Provide assessment and intervention in domestic violence, child abuse, and elder abuse situations. Work with DHS through APS & CPS Crisis Management.
  • Maintain positive working relationships, and coordinate with community resources and agencies.
  • Communicate with community partnerships through networking and community meetings such as Child Protection Team, Larimer County Women's Health Consortium and Women to Women.
  • Pre-Visit Team Based Planning - Review daily schedule and charts by care team. Documented instructions are written on the schedule, typed into the EHR appointment note, or patient cases. Ensure patient visit is more efficient by identifying: 1. Records from transitions of care are available during the patient visit. 2. Results from labs or diagnostic are in the EHR. 3. Labs or other services that are not specified in the standing orders protocols.
  • Screen patients for healthcare needs.
  • Develop a client-focused case management plan.
  • Chart documentation in EHR. Complete data entry for various programs such as WWC, CCSP & BMTF.
  • Identify patients' insurance coverage or other sources of payment for services.
  • Identify and addresses patient risk factors and/or obstacles to care.
  • Communicate the care preferences of patients, serving as their advocate, and verifying that interventions meet the patients' needs and goals of treatment.
  • Educate the patient/family/caregiver about the case management process and evaluate their understanding of the process.
  • Monitor patients according to the guidelines set by the CDC, CCGC, State and Sunrise Community Health.
  • Assist with scheduling appointment, follow-up care, referrals and medication refills.
  • Attend assigned trainings (cardiovascular, CCSP, PM, Medicaid, CGM, Insulin Pumps).
  • Assist patients with transportation needs.
  • Team Based Care - Clinical Care Collaboration - Work in collaboration and continuous partnership with providers, clinical staff, and community resources in a team approach to promote access to appropriate care in the goal of enhancing patient health.
  • Other duties as assigned.
Minimum Qualifications:
  • High School Diploma, and/or GED required.
  • Bachelor's degree (B.A.) in human services, social work and/or nursing preferred.
  • 1-2 years' related experience and/or training; or equivalent combination of education and experience.
  • Medical Assistant certification/diploma required.
  • Bilingual English/Spanish preferred.
