We are currently seeking a dynamic and talented Program/Case Manager to join our team!
The Program/Case Manager will be responsible for the coordination of administrative and discharge planning for all Pride Institute clients as well as overall management of the residential programs inclduing providing supervision/training staff, manaing daily milieu, compliance, etc. Our Program/Case Manager will gather information from the Team and our clients and formulate Aftercare Plans that continue to provide quality treatment to our clients and their loved ones and ensure best possible outcomes.
The Program/Case Manager:
- assist Director of Clinical Services with certain tasks
- provide supervision and training to staff
- ensures proper documentation of client goals and progress
- meets with clients to orient them to the facility and provides them with aftercare resources at discharge.
- completes biopsychosocial assessments, discharge summaries and other required discharge paperwork.
- assists with reviewing cases for continuing care.
- maintains periodic reports on client step-down rates and discharges.
This will primarily be a Monday through Friday, daytime hours position, and will report to our Director of Clinical Services.
QualificationsREQUIREMENTS:
- Bachelor's degree in mental health/behavioral sciences or similar
- Must be licensed by the Minnesota Board of Behavioral Health and Therapy as an Alcohol and Drug Counselor (LADC)
- Minimum of two (2) years of supervised substance abuse experience
- Skilled communicator with the ability to effectively present information to clients with differing levels of comprehension
- Proficiency with data management and analsis tools
- Must be able to pass/clear the MN DHS Background Study and pass a UA drug test. Must be fully vaccinated against COVID-19 as required by CMS.
