Penick Village Continuing Care Life Plan Community is seeking a Social Worker to join our team. The Social Worker role is an excellent opportunity for an experienced, creative and motivated individual who strives to provide service excellence.
Purpose of Job Position:
The Social Worker will provide "medically related social services to attain or maintain the highest practicable resident physical, mental and psychosocial well-being", while also adhering to state and Penick Village policies to help integrate the healthcare residents experience from preadmission through discharge. The Social Worker must maintain a desired census and waiting list, interact with the Community, implement the Community's marketing plan, travel, speak before groups, obtain resident information, complete and maintain admissions records and documents, communicate with families/responsible parties, and work with referral agencies. The Social Worker will also assist with the promotion of Penick Village and work with the Interdisciplinary Care Plan Team to provide appropriate and meaningful support for our residents.
Job Description - Social Services
Interview and compile a complete Social History on each resident in Skilled Nursing.
Keep accurate Progress Notes on all residents in MatrixCare.
Attend daily Clinical Meetings with Interdisciplinary Team (IDT) to discuss and review resident charts.
Be an advocate for residents when appropriate and ensure residents' rights at all times.
Attend Stand-up meetings, Care Plan Meetings and other appropriate meetings.
Coordinate shopping for residents as needed.
Perform all social service related duties assigned in a timely, accurate and consistent manner.
Provide care navigation to residents as transitions occur. Work with Director of Independent Living to ensure smooth transition for Level of Care Change.
Complete Minimum Data Set (MDS) portion for Social Services to include Sections C, D, E and Q with Resident Assessment Protocols.
Review Care Plans regularly to identify changes that need to be made.
Job Description - Admissions
Conduct tours of the Community for potential residents, families and community representatives from placement agencies and referral sources.
Organize marketing efforts for healthcare throughout the Community with surrounding Hospitals, Senior Centers, Home Health/Care agencies, Hospice Agencies and Assisted Living Communities.
Maintain a good public relations program that serves the best interests of the Community
Attend related functions for networking purposes as required
Follow-up on inquiries and maintain current and active waiting lists
Visit potential admissions, as necessary, to determine appropriateness.
Review potential patient information by requesting completed applications, medical information and work with Interdisciplinary Team to review medical information prior to admission.
Collect information from discharge planner and collaborate with Business Office.
