Position Summary

The Case Manager Level 2 provides program support and service coordination for people served in our Life Enrichment Day Support Services Program. Working with persons with disabilities, the Case Manager Level 2 provides community integration and inclusion through recreation, skill-building, education, and job exploration, including positive approaches to learning, systematic instruction, behavioral intervention, case management, and self-care needs. This position also provides transportation services for individuals as needed.

Hiring Bonus $1,000 ($500 payout after 6 months of employment and balance $500 after 12-month work anniversary)

Responsibilities

  • Provide comprehensive case management and person-centered support services to a caseload of around 30 individuals.
  • Be the first point of contact for service needs for individual's care team.
  • Position includes assisting individuals with personal care such as support in the restroom, completing transfers, and feeding.
  • Celebrate and inspire personal growth for the people you support as they work toward their goals.
  • Assist with training Direct Support Professionals on the needs of persons supported.
  • Create and maintain meaningful person-centered outcomes while meeting licensing standards.
  • Complete documentation accurately in the case management system.
  • Complete 245D paperwork for the people served on the caseload.
  • Coordinate semi/annual team meetings for persons served.
  • Serves as (and is qualified for) designated coordinator role under 245D
  • Supports supervisors/managers with billing-related processes.
  • Trains/mentors Case Manager I.
  • Serves as a case manager for people in multiple service lines.

Requirements

  • Have either a high school diploma or GED and 3 years of direct care experience; AA Degree and 2 years of experience; or a bachelor's degree and 1 year of experience.
  • Degree in Human Services or other related fields preferred.
  • 1 - 5 years of experience as a Case Manager.
  • Previous experience working with persons with disabilities preferred.
  • Experience working with 245D and CARF licensing is a plus; but not required.
  • Positive, dependable, and good time management skills.
  • Technology skills: Proficient with Microsoft Office software (Outlook, Word, etc.) and Case Management software - Learn software system and become proficient.
  • Familiarity or willingness to learn systems of AAC (adaptive communication) and assistive technology in areas of software, hardware, and mobility.
  • A valid driver's license and ability to pass DHS (Rule 11) and MVR background checks required.
  • Ability to lift and carry up to 25 pounds daily.
