Social Worker - Adult Day Health Program

Monday to Friday 9am to 1pm: 20 hours per week.
This position report to the ADH, Adult Day Health, Program Director. The Social Worker develops and implements a plan of care to meet the psychosocial needs of the client. This includes providing individual and group counseling to participants, families, and staff as warranted.
Requirements

1. Bachelor's degree in social work or related field from an accredited college or university.
2. One (1) year experience working with the elderly or disabled.
3. Knowledge of community resources.
4. Good communication skills.
5. Ability to read, write, speak and understand English clearly including basic math skills.
6. Evidence of maturity and ability to deal effectively with job demands and stress.
7. Ability to protect a positive image of the CCVNA HealthCare and maintain a customer-focused approach to all work.
8. Ability to work independently.
