DESCRIPTION
The Inpatient Rehab RN Case Manager is responsible for the assessment, development and implementations of the patients plan of care. In coordination of clinical review, documentation of abstracted data and communication with third party payors of all selected patients. This position involves the monitoring of appropriateness of admissions, continued stays discharge planning and resource utilization. The Case Management nurse participates in the patient care planning and transition of care.

RESPONSIBILITIES

  • Demonstrates responsibility and accountability in the performance of assessment, reassessment, development and implementation of the patient's plan of care in relationship to the discharge needs and follow-up care and in accordance with the utilization management plan.
  • Proactively communicates with the physician and other members of the health care team to determine the best options to meet the patient's needs in an efficient and effective manner.
  • Review all assigned cases within 1 work day of the admission or request for review utilizing current and accurate clinical case review criteria.
  • Promotes the most effective and efficient use of resources to achieve the targeted length of stay.
  • Assesses, collaborates and coordinates the patient's discharge planning needs and readiness for discharge with other members of the healthcare team and the patient/family/SO.
  • Complies with the departmental policies and procedures related to required processes including but not limited to documentation, interdisciplinary communication, IM delivery, teamwork, fiscal responsibility and performance improvement activities.
  • Assists Physicians in the decision of proper status placement. Informs and educates internal personnel for correct status assignment. Works with physician advisor to assure InterQual criteria are applied.
  • Refers to social worker the high-risk discharges e.g. homeless, HIV, DCF, Baker Acts, new diagnoses and difficult transfers/discharges/placements.
QUALIFICATIONS
  • Associates degree required, BSN preferred
  • 3-5 years of acute care experience
  • Hospital Nursing experience required
  • Case Management experience a plus
  • Critical Care experience a plus
  • Certification in Case Management or Utilization Review preferred
  • InterQual experience preferred
  • Fluent in Spanish a plus
