The Intensive Outreach Specialist is responsible for helping reduce group and retaliatory violence in the City of Racine by working directly with adults who are identified as very high-risk of being involved in gun violence. The Intensive Outreach Specialist is tasked with establishing trusting relationships with these adults by spending significant time with their participants, helping them reduce their risk factors, and connecting them to services, supports, and opportunities. The Intensive Outreach Specials reports directly to Director of the Department of Community Safety. Essential Duties ESSENTIAL DUTIES: Reduce retaliatory group violence in the City of Racine. Conduct outreach in the community to locate individuals identified as very high risk of being victims or perpetrators of gun violence.. Participate in custom notifications of high-risk adults and enroll in intensive case management services. Initiate contact via any means of communication with referred adults within 24-hours after assignment from the supervisor. Initiate an in-person contact with referred adults within 72 hours of assignment from the supervisor. Establish a positive and trusting relationship with each assigned participant. Develop a Comprehensive Case Plan with each assigned participant. Connect participants to services as appropriate, based on self-determined and assessed needs. Use relevant personal experiences and established relationships to influence high-risk adults\' decision-making processes to improve their outcomes. Maintain consistent contact with each participant on the case load in compliance their phase: Phase One: First 1-3 months on case load: Daily communication, 3x per week in-person contact Phase Two: 3-6 months on case load: Daily communication, 2x per week in-person contact Phase Three: 6-18 months on case load: Daily communication, 1x per week in-person contact Document case management efforts, update individual case files, and make all required entries into the data management system.
