Position Summary
The Home Visit Social Worker is responsible for assisting LCHC patients with managing their health and coordinating ongoing outreach activities within LCHC's target communities. The Home Visit Social Worker will manage a case load of patients (pregnant patients, infants or other high-risk patient groups) providing regular home visits, and aid patients in overcoming barriers to good health outcomes. Time between home visiting and community outreach will be divided according to program needs. The Home Visit Social Worker reports to the Care Coordinator Manager.
Qualifications

• Demonstrates excellent communication skills.
• Requires experience and demonstrated abilities for working in a multi-cultural setting.
• Ability to strongly embrace and personify the mission of the Lawndale Christian Health Center, through performing work in a whole person way, providing for the physical, emotional, and spiritual needs of staff members and patients. - Ability to collaborate and interact with a diverse group of health care professionals.
• Ability to organize and prioritize work appropriately.- Excellent consumer relations skills.

Required Knowledge, Experience, or Licensure/Registration

• Master's Degree in Social Work from an accredited university
• Ability to speak Spanish
• Valid driver's license and insured automobile
