REPORTS TO: Supervisor/Practice Manager SUMMARY: Performs tasks and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care under the direction of a registered nurse or licensed health care provider. RESPONSIBILITIES AND STANDARDS Responsibility I 1. Patient Care 1.2 Greet patients in waiting area, escort patients to clinical areas; escort patients to check-out. 1.3 Answer patient questions with regard to day's appointment, treatments and follow-up. 1.4 Telephone call-backs, and telephone care as directed by physician/provider and RN. 1.5 Patient counseling and education as directed by provider (verbal, brochures, handouts). 1.6 Focus on patient needs while maintaining an organized and efficient workflow as established by HQMP policies and standards. 1.7 Responsible for HPN and NYSIIS immunization registration reprinting and account updates if applicable. Responsibility II 2. Patient Encounters 2.1 Preparation of medical record in advance of appointment. This includes anticipatory tasks such as consent forms, ABNs, PHI Release, obtaining medical records from other providers/facilities, and updating of EMR, when appropriate. 2.2 Patient check-in: obtain and document chief complaint, vital signs, history, ordered CLIA waived tests; Capture charges for provision of ancillary services. 2.3 Assists patients with the completion of patient summaries, medication/allergy reconciliation at every office visit, history forms, and in obtaining appropriate treatment consents. 2.4 Prepare patient and exam room; chaperone and assist physician/provider with examination, procedures and treatments if needed. 2.5 Administer and document injections, dispense and document medications as directed by physician/provider. 2.6 Labeling and handling of labs, paps, cultures, biopsies and other specimens; Preparation of laboratory and imaging requisitions. 2.7 Assist with coordination of further patient care and follow-up, and carry out tasks as directed by RN and/or physician/provider. This may include communication with patient, hospital, labs, imaging centers, pharmacies, physician offices, insurance plans. 2.8 Clean and prepare exam room for next encounter. 2.9 Document all activities/interventions, patient/family responses, tests and treatments, in the medical. 3.0 Assumes responsibility for patient flow. 3.1 Ensure that patients have follow up testing and appointment with physicians. Responsibility III 3. General Duties 3.1 Demonstrate understanding of and compliance with HIPAA regulations, and state regulation with regard to confidentiality; Maintain compliance with infection control, OSHA guidelines, and clinical standards. 3.1a Maintain infection control measures with all patient contact. Comply with standards of Infection Control and Quality Improvement Policies and Procedures at all times. 3.1b Promotes a safe and efficient working environment by adhering to safe equipment and supply handling Policies and Procedures at all times. 3.1c Immediately reports all incidents, irregularities, unsafe conditions, equipment and/or unusual occurrences to Site Supervisor. 3.2 Keep and maintain daily log of all laboratory/imaging requisitions. 3.3 Daily reconciliation of lab reports/imaging including documentation, as appropriate. 3.4 Maintain good communication with providers and staff on issues of patient care in the spirit of teamwork. 3.5 Demonstrates knowledge of age appropriate care. 3.6 Assists with training and orientating new LPN staff. 3.7 Utilizes available education materials, program, nursing literature and resources personnel for professional development. 3.8 Demonstrates self-motivation by using unassigned time to perform duties that facilitate the smooth operations of HQMP. 3.9 Attend mandatory staff meetings and in-services training as required. Floats to other HQMP/HQUICC locations as required. Responsibility IV 4. Physical Plant 4.1 Stock exam rooms and bathrooms. 4.2 Clean and sterilize instruments. 4.3 Daily and periodic maintenance and testing of medical equipment as directed. 4.4 Organize, inventory, and maintain medical supplies - monitor medications and supplies for expiration dates. 4.5 Maintain adequate supply of laboratory specimen/transport media. 4.6 Assist office staff with filing, scanning, and other clerical needs. Responsibility V 5. Guest and Public Relations: Demonstrate regard for the dignity and respect of all patients, family members, visitors, and staff 5.1 Display a caring and courteous attitude. 5.2 Promote and contribute positively to intra-departmental and inter-departmental relationships with no more than two complaints in a six-month period. 5.3 Maintain the confidentiality of employees and departmental information with no infractions. 5.4 Adhere to departmental and hospital dress codes as observed by manager including Health Quest picture identification badge, 100% of the time. 5.5 Demonstrate willingness to assist co-workers and/or to accept additional assignments as requested to support the department efficiency as observed by the manager. 5.6 Respond to all managers' requests for information in a timely and courteous manner. 5.7 Demonstrate a high level of mental and emotional tolerance and even temperament when dealing with people; use tact, sensitivity and sound judgment at all times.
