Founded at MaineHealth Maine Medical Center (MHMMC) Portland, the MaineHealth Preble Street Learning Collaborative was established to help people in Portland who are experiencing homelessness.
Position Highlights:
Are you ready to make a real impact on the lives of individuals experiencing homelessness? As a Health Guide at the Maine Medical Center - Preble Street Learning Collaborative (PSLC), you'll provide essential support to patients with complex health and social needs.
In this role, you'll assess patients' needs, determine appropriate care levels, and assist with navigating the system. Though primarily office-based, you'll occasionally support clients in the community. Collaborate with Medical Outreach Case Managers, medical providers, and the Preble Street Health Services team to ensure effective care coordination. You'll also educate patients about resources, programs, and application processes, using your skills in de-escalation and problem-solving to help them overcome barriers to care.

Required Minimum Knowledge, Skills, and Abilities (KSAs) - "What you will need":
- Education: Bachelor's degree in community health, social work/social services, psychology or closely related field preferred OR 2-3 years directly relevant experience in a healthcare or social services setting, working with vulnerable populations may be accepted in lieu or Bachelor's degree.
- License/Certifications: N/A
- Experience: Relevant experience providing advanced knowledge of community outreach resources, Federal/State programs and skills in assessing client needs around their health and health care
