Travel Nurse RN - Home Health in Salem, OR

**Guaranteed Hours:** Not guaranteed; weekly productivity of 30 points equates to 40 hours per week
**Experience:** Minimum 1 full year of Adult Home Health or Hospice experience required
**License:** Must have license in hand
**Certifications:** CPR, BLS
**Must-Have:** Proficiency in HomeCare HomeBase EMR and OASIS, used within the last 6 months
**Job Description:**
- Provide, coordinate, and direct home nursing care based on physician's orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses.
- Perform home visits and attend periodic office meetings.
- Manage a 60-mile coverage radius, willing to travel up to 75 miles with mileage reimbursed at the standard rate.
- Conduct 5-8 patient visits per day, amounting to roughly 6-8 points.
- Handle types of cases such as Start of Care, Wound Care, and Follow-up Visits.
- Participate in a weekend and on-call rotation as required.
- Ensure documentation is completed within 24 hours of patient appointment.
- Travel between patient homes with reliable transportation, a valid driver’s license, and liability insurance.
- Be open to floating to nearby clinics if patient census affects work hours.
- Attend weekly meetings at the assigned clinic address.
- Maintain a consistent dress code in scrubs and ensure proficiency in the required EMR systems.
- Be available for the possibility of working some weekend shifts or at least one weeknight per week.
