Description
The DuPage County Health Department is proud to announce an opportunity to work in our Crisis Services - Crisis Follow Up Program. Our Crisis Follow Up Program provides supports from individuals coming from a crisis point of care. The services provided include case management, counseling/therapy, crisis intervention, and care coordination. Our program is part of a larger behavioral health program that includes outpatient services, crisis services, adult intensive services/residential services, forensic behavioral health and substance use treatment.Click to learn more about our program.
Current availability exists to work as a Crisis Support Specialist. This position will work out of our Crisis Services Center in Wheaton, ILThis full-time position works Tuesday through Saturday from 10:00 AM until 6:00 PM.

Responsibilities
  • Provides service in person, via phone, and in the community
  • Assists children and adolescent clients and their families by facilitating illness self-management skill building, identification and use of adaptive and compensatory skills, utilization of natural resources, and connection to community resources
  • Maintains a caseload and provides case management services.
  • Acts as a care coordinator, facilitation transition from a crisis point of care to ongoing services.
  • Collaborates with a multidisciplinary team and other community agencies to ensure appropriate levels of service delivery
  • Interviews and gathers information from collateral contacts, clients, and caregivers to provide appropriate resources
  • Completes client assessments, treatment plans, and participates in client engagement activities
  • Contributes collaboratively in team meetings to ensure appropriate levels of service delivery for clients.
  • Updates records in a timely manner within the electronic system
  • Maintains compliant case records including documentation of all client activity and adheres to productivity guidelines
  • Assists in providing coordination and outreach post mobile crisis response events:
    • Facilitation of inpatient placement
    • Linkage to outpatient services
  • Participates in emergency response activities as assigned
  • Provides support and coordination for the adult crisis follow up team as needed.

  • Requirements
    Bachelor's Degree in Psychology, Social Work, or a related field, and one year of related work experience, or an equivalent combination of training and work experience.
    The preferred candidate will be capable of projecting the warmth and sincerity needed to work effectively with clients; strong ability to work within a collaborative team approach; knowledge of harm reduction practices, stages of change, motivation enhancement treatment and local community resources.
    This role requires you to have a valid driver's license, along with your own personal vehicle that is reliable to drive while working in this role.
