Job Requirements / Description
PURPOSE
Provide time limited case management support services to a maximum of 40 Ontario County individuals/households meeting the HUD Emergency Solutions Grant homelessness eligibility. The primary goal is to quickly remediate a housing crisis, secure permanent housing and
assist households sustain housing; in order to reduce episode of repeat homelessness.
Job Duties
  1. Provide case management and coordination to Ontario County residents or families who are housing insecure and qualify for the HUD Emergency Solutions Grant program.
  2. Develop a working relationship with Ontario County Department of Social Services Staff, YWCA Ontario County Case Managers, property managers, agencies and landlords in the community to obtain/maintain a healthy stock of affordable apartment listing for RRH households.
  3. Conduct initial screenings/intake, assessments for each household referred to RRH program and document eligibility, services and progress in the Homeless Information Management System.
  4. Using HUD guidelines, establish eligibility for rental subsidy and accurately monitor eligibility for on-going subsidy.
  5. Conduct quality Housing inspections of all apartments/housing to ensure there are no health and safety concerns, prior to approving families/potential tenant to move into permanent housing.
  6. Communicate regularly and engage with tenant, property manager/landlord to address any concerns with income, rent or housing.
  7. Meet with tenant regularly once a month through home visit and/or phone contact and assist tenant in developing/identifying/evaluating and revising service plan/goals.
  8. Link tenant to appropriate services including employment and training services, legal assistance, substance abuse programs, mental health, children's services, child care and any other appropriate services identified by tenant.
  9. Serve as liaison between YWCA and other social agencies and community resources.
  10. Maintain accurate up-to-date electronic records, paper files, client census, financial and reports for the RRH program.
  11. Communicate and case conference regularly with supervisor, team lead and team.
  12. Demonstrates understanding of and sensitivity to cultural differences and trauma informed care.
  13. Demonstrates compliance with all appropriate HIPAA Privacy and Security Standards as defined by the YWCA of Rochester and Monroe County.
  14. Perform other duties as requested.

RELEVANT BACKGROUND
Experience and Education Qualifications
  • Associate's Degree in Psychology, Human Services or related field
  • Two (2) years' experience in crisis intervention counseling. Lived Experience is a qualifier.

Additional Requirements

TERMS OF EMPLOYMENT
This is a grant position which may not exist beyond the duration of the grant period or any subsequent renewal of it. When the grant or any subsequent renewal ends, grant employees will be terminated and their positions will cease to exist. There is also no guarantee of continued employment for the duration of the grant.
A grant-funded employee may apply for and be considered for employment in another regular position in the same manner as any other applicant. If hired into a regular position, the employee's salary from the grant position does not necessarily follow the employee into the new regular position.
