MA $46,593 - $90,910 (@.6 FTE = $27,955 - $54,546)
This position will be physically located at Dakota Boys and Girls Ranch.
Position Purpose: Liaison to Dakota Boys and Girls Ranch Treatment Facility/Dakota Memorial School, parents, outside agencies, and home school districts of the students. Provide a problem-solving service to children, families, school teams, and residential teams. The social worker will help establish family and community integration goals, attend meetings with families and youth for support, connect families to resources such as training and services, and ensure the voice of the youth and family is part of the multidisciplinary team. The social worker will provide services to youth and families that encourages education, empowerment, rebuilding of natural supports and future self-sufficiency in a supported environment.
Responsibilities:
Maintain student data and caseload information for youth residing at DBGR as part of the N&D Grant for DBGR.
Completes required paperwork/forms upon admission and discharge of the students from the treatment facility as part of the N&D Grant for DBGR.
Develop and maintain a supportive relationship with the youth and family by regularly meeting on an ongoing basis to increase family engagement.
Communicate professionally with the family regarding incidents, progress, and plans.
Contact youth and/or family during times of situational and/or environmental crisis to be a support.
Attend multidisciplinary team meetings supporting the development of treatment/safety plans for the youth to coordinate care and advocate for the youth and family.
Consider and actively try to eliminate barriers to access services for the youth and family (transportation, basic needs, therapy) through identification of needs and connecting resources.
Advocate and insist permanence (social, family, community) and discharge planning as part of the on-going treatment planning process.
Provide information regarding community-based treatment options, service opportunities, active support groups, financial support, advocacy, and training opportunities.
Complete documentation of ongoing communication with the youth, family, custodian, and providers.
Maintain survey information and data 6 months post-residential discharge.
Complete all training as assigned by DBGR and Fargo Public Schools related to this position.
Provide Social Work services based on caseload and needs of the program.
Communicates, collaborates, and cooperates with colleagues, supervisors, and students.
Provides direct counsel to students and families.
Performs casework service with individual students to correct personal, social or emotional maladjustment's related to their educational and social progress.
Coordinates or serves as a member of diagnostic teams and educational planning and placement committees and provides biological, psychological and sociological assessment information related to planning for children with adjustment problems.
Consults and collaborates with other school personnel and community resources in gathering and giving information on a case, and in establishing and planning for respective roles in the modification of the student's behavior.
Makes home school district visits with the student/guardian to support the transition back to the home school district.
Minimum Qualifications:
Bachelor's degree in social work.
ND licensure as a Social Worker or ability to obtain prior to start date.
Demonstrated special education experience.
Understanding of human behavior, legal issues, ethical issues, insurance regulations and protocols for proper referral or application for various programs.
Preferred Qualifications:
Training and experience in both inpatient and outpatient psychiatric services and chemical dependency treatment highly desirable.
Demonstrated experience working with diverse populations.
