POSITION SUMMARY:
A position requiring two essential functions at LIFE Senior Services. SeniorLine responds to individuals in need of information, assistance, and referral to community-based services in the Tulsa metropolitan area available to older adults and/or people with disabilities; this work is accomplished through telephone, e-mail, or face-to-face conversations. Independent Case Managers collaborate with clients by assessing, facilitating, planning, advocating and monitoring health and social needs, thus enabling clients to remain in their home and in the community. Administer case management within the scope of practice of the LIFE Senior Services' related programs.
PRINCIPAL DUTIES AND RESPONSIBILITIES:

EDUCATION, SKILLS AND EXPERIENCE: Bachelor's degree preferred or equivalent experience in social work, psychology, or other behavioral science field; knowledge of community resources for older adults and persons with disabilities and experience in working with one or both of these populations.
Excellent problem solving skills; ability to organize and manage a caseload in accordance with agency standards and in compliance with policies and procedures of contracted programs; ability to relate well with seniors and people with diverse disabilities in a variety of settings, e.g., home and community; ability to meet strict deadlines; ability to work both independently and interdependently; ability to work easily as part of the planning team. Ability to use a computer, especially for required reporting.
PHYSICAL REQUIREMENTS: Requires constant hand grasp and finger dexterity; frequent sitting, standing, walking and repetitive leg and arm movements, occasional bending, reaching forward and overhead, squatting and kneeling. Ability to communicate verbally with an excellent comprehension of the English language.
WORK ENVIRONMENT: The main work environment is an office setting of a non-profit organization providing a variety of services to seniors and their families. PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Independent Case Manager duties include the following:
- Utilizing courteous and compassionate active listening and assessment skills to ascertain the needs of each individual served;
- Assisting each individual by offering information about and referral to community-based services to address identified needs;
- When appropriate, engaging in more extensive exploration of the individual's needs, to include discussion of options, eligibility criteria for various programs and services, linkage with community resources, and if appropriate, referral to supervisor to assess for case management services for more in-depth or in-home assistance with planning and coordination of community-based services;
- Obtaining data concerning individuals served and referrals made and entering these data in the SeniorLine database accurately and promptly;
- Acquiring and maintaining current information about community resources, utilizing the Vintage Guide to Housing & Services and additional brochures or materials, and collaborating and sharing this information with LIFE Senior Services staff and those in similar positions in other community agencies as requested;
- Once a client is entered into the Case Management Database and assigned to a particular ICM, the ICM will:
- Complete a comprehensive assessment of client needs and capabilities;
- Collaborate with clients to develop a person-centered plan of care which includes implementation alternatives and effective use of formal and informal community resources;
- Implement, monitor and evaluate client services to achieve positive outcomes to maintain high quality, cost-effective care, while minimizing fragmented access to services;
- Assist Adult Day Health with ADvantage enrollment of ADH Clients in that program;
- Serve as an advocate for the client, with an emphasis on encouraging clients to remain as independent for as long as they are able in their own home and in the community;
- Serve as facilitator to promote communication and delivery of quality services and coordination of these services with all team members, including the client, family, providers and other agencies;
- Efficiently maintain required documentation and client records in accordance with policies and procedures of contracted programs, meeting all deadlines on time;
- Promote participation of older adults and citizens with disabilities in all aspects of community life through participation in relevant meetings, LIFE Senior Services community and related activities, etc;
- Attend required staff meetings, necessary in-service education programs and other continuing education opportunities, and when appropriate and required for certification, licensure, etc;
- Demonstrates a thorough understanding of HIPAA/HITECH laws and the related policies and procedures within the agency. Attends and successfully completes mandatory HIPAA/HITECH training at least annually.
- Participates in on-going Commission on Accreditation of Rehabilitation Facilities (CARF) accreditation procedures of LSS.
- Attends and participates in staff meetings, in-services and projects and committees as assigned. Completes required training as assigned in Relias.
- Adheres to and support the facilities policies, practices and procedures.
- Accepts assigned duties in a cooperative manner and perform all other related duties as assigned by Supervisor.
- Participates in special projects and performs additional duties as required or assigned.
