This is a great opportunity to apply your clinical skills while treating patients with persistent mental illnesses as a part of a multidisciplinary team.
Case Managers are responsible for coordinating and assuring the continuity of services for each patient on their caseload. This includes working with each patient and caregiver(s) to coordinate care planning, complete a comprehensive assessment of needs, develop a Case Management Plan, monitor the effectiveness of it, plus ensure resource linkage, referral, and advocacy. May also coordinate care and maintain the medical record within the agency.
QUALIFICATIONS: Bachelor's degree in Social Work, Psychology, Human Services or other related field preferred but not required. One - three years of experience in mental health preferred, or lived experience. Valid driver's license, insurance and vehicle required.
