Title: Intake Coordinator
Unit: Behavioral Health Services
Reports To: Senior Director of Behavioral Health Services
Status: Full-Time; Regular; Non-Exempt
Location: Long Island City, Queens
Days/Hours: Monday to Friday 9:00am to 5:00pm
Salary : $27.48 to $32.97 (approximately $50,000 - $60,000 annually)
Position Summary:
The Intake Coordinator is a member of the Behavioral Health (BH) services team which includes Treatment Services and Medication Assisted Treatment (OASAS/substance use), the Better Living Center MHOTRS program, The Drop In Center (HIV+), and the Care Management Unit (Health Home care mgt.). The Intake Coordinator reviews internal and external referrals and is the initial point of contact for all clients. S/he follows cases from the date of receiving the referral through either admission or closure of the referral, and develops relationships with existing and potential referral sources. The Intake Coordinator also has administrative responsibilities for Behavioral Health Services such as being responsible for the main clinic phones, scheduling appointments and providing reminders.

All client interactions will be recorded into Fortune Society's databases, and therefore the ideal candidate must have computer proficiency (knowledge of MS Office, Excel, and electronic health record systems) and comfortability with core technology such as Zoom, WebEx, etc.

Essential Duties and Responsibilities:
- Establish rapport with clients and accurately obtain background and demographic information to initiate BH services;
- Track all internal and external referrals and maintain correspondence with referral sources and clients during pre-admission period;
- Screen referrals for priority per OASAS or OMH guidelines;
- Conduct pre-release screenings for DOCCS and DOC as well as pre-and post-release housing screenings as needed;
- Facilitate weekly orientations for newly enrolled participants as needed;
- Conduct evidence-based screenings with clients to identify appropriate levels of care for treatment;
- Complete thorough documentation of screening results, preliminary diagnosis, level of care determination, and intake decision;
- Manage client schedules for the treatment services clinic in Accumedic and reconcile clinical schedules with claim volume; accurately enroll new clients into Caseworthy, conduct preliminary insurance checks, and complete intake and consent process;
- Conduct and perform data entry for GPRA and SAMHSA Cross-Site surveys/evaluations at intake, 6-month follow-up, and discharge, maintaining appropriate follow-up rates;
- Schedule initial health screenings with the RN;
- Participate in Multidisciplinary Team meetings (MDT), collaborating on client/counselor assignment;
- Screen participants at intake for appropriateness of referrals to other Fortune Society programs;
- Notify clients of cancelled appointments;
- Manage the board report process and other data analysis projects as requested by senior management; liaise with EQI as appropriate to complete projects;
- Answer main clinic number, manage faxes and mail for the Behavioral Health division;
- Other duties as assigned;
Qualifications:
- CASAC-T/CASAC required
- Bachelor's Degree, or High School diploma with commensurate experience in the field preferred;
- Minimum of 1 year of experience as an administrative assistant or coordinator in an outpatient clinic setting, preferably mental health and/or substance abuse related;
- Some work or internship experience with individuals experiencing psychiatric issues, trauma or incarceration a plus;
- Strong computer and organizational skills, including proficiency in Microsoft Office;
- Bilingual is a plus;
