OHI is switching to four, ten-hour shifts in January, 2025. Candidate must be able to commit to this schedule in 2025.
Position Summary
In collaboration with Chief Medical Officer (CMO), the Family Planning Services Coordinator is responsible for managing programs surrounding family planning, pregnancy and centering. This includes planning and organizing the group sessions, participating in community outreach events, and helping to recruit patients into the program through direct interactions, social media and other forms of outreach.
Responsibilities

  • Ensures that Family Planning and Centering operations run efficiently and effectively, including patient scheduling, billing, and coordination of these services.
  • Develops and delivers Centering Pregnancy and Family Planning specific training to support staff (LPNs, CMAs and CECs)
  • Coordinate OHI's Centering program including identifying patients for groups, managing group schedules and maintaining and reporting data and outcomes from each group. Assist with facilitating Centering groups.
  • Serves as the Centering coordinator for Centering Pregnancy and Centering Parenting.
  • Identifies opportunities to promote Centering to patients, clinic staff and to the community.
  • Oversees the planning and set-up for groups sessions and ensures the participating staff members involved are at their active stations.
  • Assists with data collection and report preparation to meet requirements. .
  • Meets with pregnant patients at 36 weeks to review educational packet and assist patient with needs associated with upcoming childbirth.
  • Completes a PRA Form on every prenatal patient at first prenatal visit, Enters PRA Form online within 24 hours of completion of OB reg. Participates in PRA data review process. Coordinates with Billing department to ensure all PRA information is completed.
  • Lead for Family Planning initiatives. Develops family planning training for all staff in compliance with program standards. Develops and implements a system to ensure that every eligible patient at any OHI site is provided the opportunity to participate in the program, ensuring access for all while enhancing revenue stream.
  • Involvement in ensuring completion of Family Planning templates in all departments.
  • Meets regularly with OHI leadership to review clinical, financial, and operational performance of Family Planning and the Centering Program.
  • Meets regularly with the Family Planning League of New Jersey to provide updates on our program.
  • Works with Data Team to provide Family Planning template updates and Education Team to provide education to all staff regarding Family Planning initiatives.
  • Develops strategic plan and Monday Board for Family Planning initiatives including managing expenditure of grant funds.
  • Collaborates with Data Department to develop Family Planning Data Reports and PowerBI Dashboards to keep track of progress.
  • Works in conjunction with department management to analyze and expedite performance improvement and maximize outcomes within the department.
  • Builds practice image by collaborating with patients, community organizations, and employees, enforcing ethical business practices.
  • Maintain the quality of service by establishing and enforcing organization standards.
  • Improving strategies to reach the OB program's goals (i.e. reducing low birth weight babies, improving early entry into prenatal care, ensuring that patients are not lost to follow-up, etc.).
  • Protects patient privacy and maintain accurate medical records.
  • Must be capable and analysis routine in-office testing.
  • Participates in and supports accreditation, compliance, and regulatory activities of OHI.
  • Completes all mandatory in-services.
  • Adheres to Corporate Compliance policies.
  • Is highly accountable for one's own productivity. In order to maintain transparency, all projects must be represented clearly on a project board in Monday.com. Responsible for ensuring the board is always updated and accurate and that information is accessible to those that need it.
  • Other duties as assigned.
Education/Experience/Licensure
  • Experience in Centering and Family Planning Services.
  • One to three years' experience in a community health care environment, medical center or other related field.
  • Bi-lingual (English/Spanish) is preferred.
  • BLS for providers through the American Heart Association, BCLS, ACLS and PALS certifications are required.
  • Leadership capabilities and ability to adapt in fast-paced changing environments.
