Required Certifications and Skills:

Key Responsibilities:

  • Evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration. Recommend therapy changes to the physician and facility staff as appropriate.
  • Maintain patient/resident database, including current and past medical history, clinical and laboratory data, and concurrent medication (including over-the-counter and home remedies).
  • Ensure appropriate laboratory tests for monitoring certain high-risk conditions are ordered, evaluated, and discussed with the healthcare team.
  • Collaborate with the healthcare team to provide clinical and technical support as needed.
  • Document all communication with healthcare professionals, patients/residents, and their family members in the medical record.
  • Implement policies and procedures for the safe procurement, storage, distribution, use, and disposal of drugs and biologicals.
  • Obtain and maintain all required documentation per federal and state regulatory agencies and policy.
  • Inform the Pharmacy Manager of medication-related errors.
  • Assist facility/agency staff with procedures for the safe and effective procurement, maintenance, administration, and disposal of drugs.
  • Perform other tasks as assigned.


Qualifications:

  • Bachelor's Degree in Pharmacy (Pharm D preferred)
  • One year of experience in long-term care
  • Licenses/Certifications as required by government regulations and business needs
  • Detail-oriented, goal-oriented, team player with strong oral communication skills
