Physician Affiliate Group of New York (PAGNY) is adding a Chief of Ambulatory Care to the Ambulatory Care group at NYC Health + Hospitals/Kings County. The Hospital is part of NYC Health + Hospitals. Kings County Hospital operates a world-renowned Level 1 Trauma Center, one of only three in Brooklyn, which serves 2.6 million residents of Brooklyn and Staten Island.
The hospital maintains a strong academic affiliation with SUNY Downstate Health Sciences University to maintain its high healthcare delivery standards.

The roles of the Ambulatory Care Chief are:
- to oversee and improve the outpatient clinical services;
- to oversee the operational services, in conjunction with the Ambulatory Care administrative lead;
- to represent Ambulatory Care facility-wide.
MAJOR TASKS, DUTIES AND RESPONSIBILITIES:
1. Provide leadership to the clinical providers in Ambulatory Care
- Set expectations for attendings, house staff, and providers
- Implement evidence-based care guidelines and best practices
- Support quality and performance improvement activities (including operational activities such as Access)
- Participate in the assessment process of providers and clinic chiefs
- Participate in the recruitment of new faculty and staff
2. Provide educational and academic leadership
- Support clinic-based research and scholarship
- Mentor clinic chiefs
3. Oversee improvement initiatives in outpatient services
- Support Ambulatory Care Leadership Council initiatives
- Support Central Office initiatives (e.g. eConsults)
- Support Managed Care expansion/QARR initiatives
- Support ambulatory care patient growth and Managed Care enrollment
- Optimize Population Health, Chronic disease management and Value Based Programming
- Increase access across all services
- Enhance quality and outcomes
- Improve Care Experience to 50th percentile
- Increase staff and community engagement
- Address space utilization in ambulatory care
- Address patient advocacy and patient rights issues
- Support and advise on template management
- Support enforcement of compliance issues
- Risk management/quality case reviews
- In collaboration with the Chief Growth Officer, supervise the Ambulatory Care Administrator and Director of Nursing and participate in annual performance assessments
4. Participate in meetings
- Represent the facility on the NYC Health + Hospitals Ambulatory Care Leadership Council Executive Committee
- Lead facility ambulatory care triad leadership meetings, Ambulatory Care Clinical Service Line meeting
- Population Health and Chronic disease management
- PCMH, ACO
- Hospital-wide committees (e.g. Cabinet, Drugs and Formulary committee, etc.)
- Finance
5. Oversee performance metrics
- Depression
- A1c < 8
- Blood pressure control
- Unplanned admissions for patients with multiple chronic conditions
- 7-day follow-up after hospitalization for mental illness
- CG-CAHPS
- Breast cancer screening
- VBP, HF, CRG, HCC Risk, External Utilization and Enrollments Performance
- Financial closure of ambulatory visits
6. Participate in facility-wide activities
- Quality improvement and patient safety
- Regulatory surveys and reviews
- Joint commission preparation
- QAPI Board Report preparation
Qualifications
- ABMS Board Certified in an Ambulatory Care Specialty Field
- Active New York State Medical License
- Graduation from an accredited medical school
Wages and Benefits include:
Annual Base Salary: $340,475.20 based on a 40-hour work week.
The annual total value of the compensation package is estimated at $374,522.20**, which includes the baseline salary, 401(k) contribution, and other factors as set forth below:
- 401(k) Company Contribution (subject to IRS contribution limits):
- Employees are immediately vested in a 3% company contribution of base earnings. No employee match is required.
- After one year of service, employees receive an additional 7% company contribution of base earnings. No employee match is required.
