About You:
We provide supportive trans services that go beyond medical care, including various programs, support groups, and outreach & navigation initiatives that are for, by, and of us. The focus of this case management program, housed within our behavioral health team, is to: 1. Work with unhoused and unstably housed trans people to help them take the necessary steps to access gender-affirming care and 2: provide assessment, referral, and support services to help them access essential medical, behavioral health, housing, and other psychosocial services. Ultimately, this program will support our clients in accessing gender-affirming care while achieving long-term housing and stability.
Role Summary:
The Case Manager - Trans Services is part of a multidisciplinary team that includes medical providers, nurses, medical assistants, social workers/therapists, and community health workers. The Case Manager - Trans Services is responsible for conducting street and site-based outreach to enroll unhoused transgender and gender-nonconforming clients into a program of ongoing assessment, navigation, and linkage to medical and psychosocial services, with a particular focus on linking clients to housing and gender-affirming care, including surgery. This position will carry a caseload of unhoused transgender and gender-nonconforming clients within the agency.

Essential Functions:
- Street and site-based outreach
- Intake and assessment of new clients
- Case management and advocacy services for clients
- Basic education and advocacy around gender-affirming care, as well as general health education
- Supportive counseling and the formulation of service goals with clients
- Conducting individual counseling sessions focused on housing and gender-affirming care access
- Providing referrals to external services and monitoring the status of those referrals.
- Completion of necessary documentation in a timely manner, including UOS reports and treatment plans
- Participation in case conferences, monthly staff meetings, and in-service trainings
- Participation in, and compliance with, agency quality assurance, peer review, and clinical consultation
- Collaboration with Care Services Team to identify new clients' needs and develop programs accordingly
- Client outreach and navigation as needed
- Other duties and tasks as assigned
- BA in a health sciences or social work-related field or AAA/CMH/CHW or comparable work experience
- One year of experience providing counseling, advocacy, education and service coordination and/or outreach work to disenfranchised communities
- Knowledge and expertise in navigating resources in San Francisco
- Experience working with the LGBTQ community, specifically the Trans and GNC community
- Knowledge of challenges facing individuals living with HIV /AIDS, STIs, substance use, mental health, social service, and the probation system
- Familiarity with issues in communities of color
- Knowledge of harm reduction philosophy, client centered counseling, motivational interviewing techniques, and trauma informed care.
- Must be adaptive, flexible and a team player
- Strong verbal and written proficiency in English
- Experience working with diverse sexual/gender orientations, the homeless, limited English speakers, and/or multiple-diagnosed individuals
- Demonstrated excellence in attendance and reliability, interpersonal, organizational, verbal, and written communication skills
- Familiarity and experience with Microsoft Word and Outlook programs
- Detail-oriented with strong follow-through skills, including attention to, and respect for, detail
- Must be able to sit, stand, and walk for extended periods of time
- Must obtain TB clearance (within past 12 months) and complete background check upon hire.
- Proof of currency with vaccinations, including latest Covid-19 vaccine with at least one booster, and seasonal flu.
- Lived experience as a member of priority populations.
- Familiarity with data systems such as ARIES, EPIC, and electronic health record systems
- Experience working in an FQHC or other type of Community Health Center.
