Position: Case Manager
Location: In-Person, Port Hueneme, California
We're looking for someone to join our team as a Case Manager who is responsible for collaborating with the members of the treatment team to facilitate the coordination of appropriate services that are consistent with the resident/client's plan of care, help achieve their optimal level of independence and enhance quality of life. This individual is actively registered in California as a RADT, SUDRC, or RAC by a Certifying Organization and is working towards full certification. As a member of the multidisciplinary treatment team, they develop aftercare plans that integrate medical, social, and resource issues that impact individual clients. This is a safety sensitive position.
Duties may include but are not limited to:
  • Conduct psychoeducational groups for clients with an emphasis on relapse prevention, aftercare planning, and community support
  • Ensures consistent and reliable documentation of case management activities in compliance with all organization and department standards
  • Develop thorough, individual continuing care plans in alignment with treatment team recommendations and client needs
  • Meet with clients regarding individualized continuing care plan and address planning needs and concerns
  • Educate clients on the importance of follow up care in their recovery
  • Complete discharge planning progress notes and discharge summary in a timely manner
  • Facilitate continuing treatment for discharging residents that may require additional services such as PHP/IOP in distant locations
Requirements:
  • High School Diploma or GED Equivalent Required
  • Active registration as a Registered Drug and Alcohol Technician (RADT), Substance Use Disorder Registered Counselor (SUDRC), or Registered Addiction Counselor (RAC) by an approved DHCS certifying organization
  • Current enrollment in an academic addiction studies program
  • Valid California Driver License with a good driving record (in the Company's sole discretion)
Additional Qualifications:
  • CPR and first aid certification
  • Experience dealing with clients and documentation in a hospital, substance abuse treatment facility, and/or mental health facility preferred
  • Regular local travel among company locations.
  • Must fulfill pre-employment conditions: tuberculosis screening (annually thereafter), background check (including criminal and driving record check), drug screening, and reference checks. Ongoing monitoring and screenings during employment in accordance with the law and company policy.
  • Must be able to work flexible hours, including days, evenings, weekends, and holidays to meet 24/7/365 facility needs
