Key Responsibilities:
- Provide integrated services that are welcoming and culturally competent, addressing physical health care, behavioral health care, and social needs.
- Conduct health promotion activities, including vital checks, health education, and lifestyle interventions such as substance use prevention and nutritional counseling.
- Assist clients with medication adherence, crisis planning, and accessing community resources to enhance their quality of life.
- Facilitate peer support groups, including Wellness Recovery Action Plan (WRAP), to empower clients in their recovery journey.
- Collaborate with multi-disciplinary teams to develop and implement care management plans that integrate physical and behavioral health needs.
- Maintain accurate clinical records, comply with agency policies and accreditation standards, and ensure timely completion of required forms.

Qualifications:
- High school diploma or GED required; additional training in mental health recovery and wellness preferred.
- Obtain ODMHSAS Peer Recovery Support Specialist and Wellness Coach certifications within 60 days of hire if not already certified. Certification as a WRAP facilitator preferred.
- Proficiency in MS Word and general computer skills; excellent interpersonal, verbal, and written communication skills.
- Flexible schedule for community-based services, including potential weekend and holiday availability.
- Valid Oklahoma driver's license, automobile liability insurance, and recent TB test.
- CPR, First Aid, Therapeutic Options, Co-Occurring Training, and Trauma Informed Training (within 60 days of employment).
